2. Download and print out the petition form or see the attachment. Total 6 pages.
3. Fill out and sign the form in the middle area of the 4th page.

5. Or follow the instructions on the 6th page.
6. If you want to get the petition signed by more people and collect them, fill out the bottom section of page 4 “Declaration of Circulation” and get form signed by those people and mail all the signed forms (including the declaration form you signed) to the same address.
7. If you don’t have a printer at home, you can go to library to get the form printed out.
